Medical Document

Similar to a prescription, a Medical Document must be signed by your healthcare professional and then mailed or faxed to the MyMedi.ca Care Team. A completed Medical Document is required for patients to obtain medical cannabis.

If you are transferring your Medical Document from another licensed producer, please register online and simply contact your licensed producer to request a transfer.
You do not require a new Medical Document. Any patient in Canada with a valid Medical Document can reigster and transfer to MyMedi.ca

Registration

If you have registered online, you’ve already completed this step. If you do not wish to register online, download a registration package here and have your healthcare professional send us the completed package by secure fax along with your Medical Document.

All patients transferring from another licensed producer are required to register online or complete a registration package.

If you have requested that an adult caregiver be permitted to order and receive medical cannabis on your behalf, a Caregiver Form must be submitted.

Registered patients are required to complete an Amendment Form for any change
